Local anaesthetic is given to numb the area of your scalp that will be treated. Then your surgeon makes tiny cuts in the bald area (the recipient site) and carefully transplants healthy hair follicles. Hundreds or even thousands of these grafts may be transferred in one session.
The transplanted follicles are anchored in the scalp and can grow and survive for your entire life. The new grafts may not appear dense at first, but they should fill in after about nine months and should look very natural. It is important that you don’t expect a full head of hair – the amount of growth will depend on your remaining hair, which may not be thick enough to provide a luxurious crop. Moreover, the colour and texture of your existing hair will determine how well your grafts take to their new surroundings.
You will be given instructions about how to care for your scalp after the treatment. Your doctor will tell you not to touch the grafts or wash your hair too vigorously, since this could damage the new follicles. It is also a good idea to avoid sun exposure and use sunscreen if you go outside. Your doctor will probably advise you not to smoke, since this increases your risk of complications.
During a hair transplant, surgeons work with binocular stereo-microscopes to ensure that the grafts are taken from a safe part of your scalp that will not grow any more than it does now. They will close the area with stitches that will be invisible when your hair grows back.
They then microscopically divide the strip of hair-bearing scalp into a number of follicular groupings, each containing different numbers of hairs. The smallest follicular units, called punch grafts, contain about 10-15 hairs each; mini-grafts contain two to four; and slit grafts have about 30-40 hairs each. They are then inserted into the recipient site, where they will form new hairs that blend in with your other hair.
In some cases, the medical practitioner will insert balloon-like devices under the skin of a hair-bearing section of your scalp and gradually inflate them with saline solution over several weeks to encourage the scalp to grow more tissue. Once this is done, the extra tissue is removed and the grafts are inserted into the treatment area.
The final result should blend in with your other hair and should not be obvious. There are some people who experience inflammation or infection of the new grafts, which usually clears up with antibiotics. There are a few other risks associated with hair transplantation, including the possibility that the new hair will fall out again once it starts growing, although this is rare.
